Appointments
Please feel free to call us during any of our listed office hours if you wish to schedule and appointment. You can also use the appointment form located on this website located in the main menu. If you are a new patient, please fill out the new patient form, also located in the main menu.
Financial
We will do everything we can to make sure you can afford the treatment you need and want. We are more than happy to submit any insurance forms you have for us. At the front desk, we accept check, cash, or any major credit card for means of payment.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
Please alert us within 24 hours of your scheduled appointment if you wish to make a cancellation. We ask this in case there may be another patient we fit in your absence. If you do not provide us with a sufficient notice to your appointment cancelation a fee may be charged.
